Forms

Everything you need in one place. This page brings together the forms we ask parents and carers to complete, so you can find them quickly rather than having to contact the office.

Many of our day-to-day forms are now completed through Arbor, our school management system. Where that is the case, we have said so and linked you straight to it. Anything that still needs a paper form can be downloaded here, printed and returned to the school office.

Joining Ashdon

  • Reception admission application — applications for a Reception place are made through Essex County Council.

  • In-year admission application — for a child joining us at any other point in the school year, also through Essex County Council.

  • Pupil data collection sheet — contact details, medical information and other essential information we hold for your child.

  • Home–school agreement — sets out what you can expect from us and what we ask of you and your child.

  • Photography and media consent — permission for us to use photographs of your child in newsletters, on our website and on social media.

Keeping your details up to date

Please tell us straight away if anything changes. Having the right contact details is essential in an emergency.

  • Change of contact details — update your telephone number, email address or emergency contacts in the Arbor Parent Portal.

  • Additional emergency contacts — to add another named adult we can contact if we cannot reach you.

  • Change of name or address — please contact the office, as we will need to see supporting documentation.

  • Parental responsibility information — if there are court orders or arrangements we need to be aware of, please speak to the office directly.

Attendance and absence

  • Reporting an absence — please let us know before the start of the school day if your child will not be in.

  • Request for leave of absence in term time — must be submitted in advance. Please note that leave in term time cannot usually be authorised.

  • Medical appointment notification — to let us know in advance about an appointment during the school day.

Medical and welfare

  • Administration of medicines consent — we must have written consent before we can give your child any medicine in school.

  • Allergy and anaphylaxis information — please tell us about any allergy so that we can put the right arrangements in place.

  • Asthma and inhaler consent — so that we can keep an inhaler in school and support your child to use it.

  • Individual healthcare plan — for children with a longer-term medical condition. Please speak to the office to arrange one.

  • Dietary requirements — so that our kitchen can plan meals around your child’s needs.

Lunches, payments and wraparound care

  • School meals — meals are booked and paid for through Arbor. School lunches cost £3.30 per day.

  • Free school meals — check whether your child is eligible through Essex County Council. It is always worth checking, as eligibility also brings additional funding to the school.

  • Wraparound care — breakfast and after-school care is provided by KidzActive and booked directly with them.

  • School milk — to register your child for milk at morning break.

Trips, clubs and activities

  • Educational visit consent — issued for each trip, with details of the visit and what your child will need.

  • Local area consent — a single form covering walks in the village and visits to All Saints Church.

  • Swimming consent — including information about your child’s swimming ability and confidence in the water.

  • Clubs and activities — places in our lunchtime and after-school clubs are booked at the start of each term.

Getting involved

We are a village school and we rely on the support of our families and our community. If you can spare some time, we would love to hear from you.

  • Parent volunteer form — to help in school, on trips or with reading. All volunteers are subject to our safer recruitment checks.

  • Friends of Ashdon — join our parent and carer group and help with fundraising and school events.

  • Becoming a governor — to register your interest in joining our governing body.

If something goes wrong, or you need information

  • Complaints form — please read our complaints policy first, as most concerns are resolved quickly by speaking to your child’s class teacher.

  • Request for paper copies — we will provide a printed copy of anything on this website free of charge.

  • Subject access request — to ask for a copy of the personal information we hold about you or your child.

  • Freedom of information request — please email the school office with the details of your request.
